Quality Care Membership grants you access to the very best in preventive care at a price that is affordable and makes budgeting for your necessary dentistry easier and more predictable. Your membership will cover your annual checkup exam and x-rays and cleanings for one year from the time you purchased it. Any dental work you should require while your plan is active will qualify for a 10% discount when payment is received at the time of service.
Certain conditions must be met in order keep this program viable and fair to all participants. Keeping appointments: You will be expected to respect the time reserved for you with your hygienist and doctor. If you cannot make an appointment please call us and speak with a team member with at least 2 business days notice to request a new appointment time. These requests cannot be handled electronically, on evenings, weekends or during office closures. Your membership allows for one re-scheduling of a hygiene appointment without penalty provided we receive sufficient notice. Subsequent cancellations will result in forfeiture of that benefit. A late cancellation of an appointment made with the doctor will result in forfeiture of any discount on the treatment.
Your membership is good for one year from the date of initiation. Failure to utilize any benefit will be considered a voluntary forfeiture. No refunds will be issued for benefits not used. Third party financing can be used to pay for restorative dental treatment under the QCM program but no discounts will be applied.
The Quality Care Membership fee for healthy gums 2024 is $462 and includes two cleanings, annual exam and x-rays, and a 10% courtesy* on other dental work when payment is received at the time of service.
*Certain exceptions such as Invisalign, lab fees and appliances
